Travel Insurance for Iran

Insurance is mandatory for Iran, a rule enforced at the airport when you arrive. This is not a suggestion. It is a visa requirement that has been in place for over a decade. The policy must explicitly list Iran as a covered country. You can buy a basic plan at the airport for visa compliance. But that tends to cause delays and offers very limited coverage. Citizens from a handful of neighboring countries are exempt. For most travelers, arranging this before you fly is a non-negotiable part of the process.

Most Western insurance companies exclude Iran due to US/EU sanctions. World Nomads, Allianz Travel, and most US-based insurers explicitly exclude coverage. IATI Insurance (European), True Traveller (UK), and Saman Insurance (Iranian) are among the few providers that cover Iran. US citizens should consult OFAC compliance guidance before purchasing any Iran-covering policy.

Healthcare in Iran

What to expect if you need medical care

Healthcare in Iran's major cities is surprisingly good, and doctors often speak English well enough for consultations. The costs are a major advantage, with an emergency room visit averaging around a hundred dollars and a full hospital day costing less than two hundred. That said, you will find facilities outside urban centers are more basic. A significant practical issue is that international sanctions cause frequent shortages of specific medicines and some medical equipment. This means for complex or ongoing conditions, you might need to be evacuated to a country like Turkey, which introduces a whole other layer of complexity.

What Your Policy Should Cover

Country-specific considerations for Iran

Given the risks here, your policy needs to be solid. Medical evacuation coverage is absolutely critical because sanctions make arranging and paying for an emergency flight out extremely difficult. You will also want high medical limits, not just for evacuation but because of specific health risks like Crimean-Congo hemorrhagic fever, which has a high risk in spring and summer. Since road accidents are a leading cause of death for visitors, ensure your coverage includes this. Finally, given the moderate year-round risk of diseases like rabies and hepatitis A, a policy that covers treatment for these is wise.

General_travel: US sanctions mean most Western insurance companies will NOT cover Iran. World Nomads, Allianz, and most US-based insurers explicitly exclude Iran. IATI Insurance (European) and Saman Insurance (Iranian) are among the few options.
Driving: Road traffic accidents are the leading cause of death for foreign visitors. Night driving outside cities is dangerous.
Rural_travel: Healthcare facilities outside major cities are limited. Carry essential medications as sanctions cause drug shortages.

How Much Coverage Do You Need?

Our recommendation based on Iran's healthcare costs

The recommended $100,000 coverage might seem high given Iran's low treatment costs. But it is justified by the evacuation risk. While a hospital day is affordable, the process of medically evacuating you to a place like Turkey is astronomically expensive and logistically fraught due to banking sanctions and potential airspace issues. That $100,000 buffer is there to handle that scenario, which is a high-risk possibility. The $50,000 minimum is the bare legal floor. But it could be quickly exhausted if a serious evacuation becomes necessary.

Making a Claim in Iran

Tips for smooth claims processing

Documentation Required: OFAC sanctions prohibit US persons from processing Iran-related insurance payments without authorization. Banking barriers between Iran and Western banks are severe. Recommended strategy: purchase both a local Iranian policy (Saman Insurance, ~$20-25) for visa compliance and local claims, plus an international policy from IATI or True Traveller for higher limits and evacuation.
